OTTAWA -- A new study has found that nearly a fifth of the country's overworked and over-stretched nurses acknowledged making mistakes in medicating patients "occasionally'' or "frequently.''
There's plenty of dosage systems that can eliminate most or all of the errors. Most errors occur from improper charting and a lacking continuity of care. Staff shortages can create problems too.

Some of the technology available to eliminate medication errors are held up by those very same people who make the errors. They don't want to change and can make a big enough fuss that it halts the progress. I've seen it happen with this very same issue.
